Nurse Practitioner Purpose of Job: - Responsible for the delivery of medical care service to pre-designated group of enrollees in collaboration with designated physicians. - Provide continuing primary care management. - Responsible for establishment and maintenance of effective relationships with members. Key decision-makers, facility staff, and collaborating physicians. % of Time and Major Responsibilities 55% Primary Care Delivery: - Deliver cost effective, quality care to assigned members in collaboration with their assigned physicians. - Manage both chronic and acute conditions effectively while remaining compliant with visit requirements. - Perform comprehensive assessment and document findings in a concise/comprehensive manner that is compliant to OBRA regulations. - behavioral issues alone or in a collaboration with physician or specialty provider. - Facilitate the implementation of the plan of care by engaging the facility staff and ensuring their compliance. - Evaluate the effectiveness, necessity, and efficiency of the plan of care and make revisions as needed. - Utilize practice guidelines and protocols established. 30% Care Coordination - Assess the medical necessity/appropriateness of ancillary services and interface directly with the providers and the case managers. - Understand the benefits and associate policies and procedures, and articulate them effectively to providers, members, and key decision makers. - Avoid use of inappropriate, high cost, poor outcome services, such as hospitalizations, by utilizing other resources or benefits. - Act as a resource for the case managers and provide them with all the requested information in a timely manner. 15% Program Enhancement - Communicate effectively with physicians, members, key decision makers, nursing facilities. staff and other provider groups. - Establish regular communication, both internally and externally. - Use communication skills to address organizational and public relations issues. - Exhibit original thinking and creativity in the development of a new and improved methods and approaches to problems. - Function independently and responsibly with minimal need for supervision. - Demonstrate initiative in achieving individual, team, and organizational goals and objectives. Physical Requirements: - Ability to lift up to 40 pounds - Transporting equipment, supplies, and paper information (books, documents) requires packing the carrying case or bag, pulling the case if on wheels, carrying the case by the handle or shoulder strap. - Ability to carry the case up and down stairs and pull up a ramp. - Ability to freely carry laptop and or book and paper in arms or hands. - Ability to push and/or pull up to 30-40 pounds including up and down ramps. - Bending, squatting- up and down motion several times a day. - Sitting and standing throughout the day.
